Job Summary

We are seeking an Admissions Coordinator Specialist to join our team as an integral member of the Intake Department. This role contributes significantly to increasing revenue and reputation by converting referrals to successful admissions while providing excellent customer service and maintaining high standards of quality.

Key Responsibilities
  • Assess, triage, and process referrals to successful admissions in a timely, accurate, and professional manner, adhering to VNA Health's policies/protocols and CMS regulations.
  • Collaborate with departments to meet relevant requirements, protocols, and regulations, ensuring seamless operations.
  • Communicate effectively with patients/families to educate about and facilitate for successful home care services.
  • Enter all pertinent data in electronic systems with high accuracy, timeliness, and completeness.
Requirements
  • Bachelor's degree preferred, with office and/or customer service experience required - experience in home health, hospice, personal care services a plus.
  • Excellent critical thinking skills to assess situations and recommend solutions.
  • High attention to detail and strong organizational skills.
  • Exceptional communication skills, including strong listening skills and ability to respectfully convey information to diverse constituents.

  • Location: Onsite in Santa Barbara, with a schedule of Sunday-Thursday 8-5PM.
