Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients' physical and emotional needs, develop and implement individualized care plans, and coordinate with interdisciplinary teams to ensure seamless care delivery.
- Direct Patient Care: Provide direct care to patients, including administering medications, performing procedures, and monitoring vital signs.
- Education and Health Promotion: Educate patients and families on health promotion, disease prevention, and management of chronic conditions.
- Collaboration and Communication: Work effectively with healthcare teams, patients, and families to ensure clear communication and coordination of care.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Education: Graduation from an accredited school of nursing.
- Experience: One year of acute care RN experience and telemetry skills required.
- Licensure: Licensed and currently registered nurse in the State of Connecticut.
- Certifications: Current BCLS certification or must obtain within one month of hire.
This role requires prolonged standing, walking, and use of sight, hearing, and touch. May be exposed to infectious diseases and physical, mental, and emotional illness as well as end-of-life conditions and death. Moderate stress and risk of injury from patient care.
