Nurse Practitioner Primary Care Opportunity

Nuvance Health is seeking a highly skilled and compassionate Primary Care Nurse Practitioner to join our team. As a Primary Care Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to our patients in a fast-paced and dynamic environment.
Job Responsibilities:
- Provide primary care to patients, including health promotion, disease prevention, and management of acute and chronic illnesses.
- Conduct thorough physical exams, take medical histories, and order diagnostic tests as needed.
- Develop and implement individualized treatment plans, including medication management and referrals to specialists.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Stay up-to-date with the latest medical research, guidelines, and best practices in primary care.
Requirements:
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
- Current APRN license in the state of Connecticut.
- Board certification in a primary care specialty (e.g., Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner).
- Minimum 2 years of experience as a Primary Care Nurse Practitioner.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
