Job Summary:

The Nurse Practitioner will provide high-quality primary care to seniors in a high-touch, person-centered care model. The ideal candidate will have a passion for delivering exceptional care and a strong commitment to customer satisfaction.

Responsibilities:
  • Assess, diagnose, and manage chronic and acute conditions
  • Provide health education and develop treatment plans
  • Order and interpret lab and diagnostic testing
  • Maintain accurate and compliant documentation
  • Collaborate with local and regional operations and clinical leadership
  • Support value-based care initiatives
  • Provide an excellent customer experience
Requirements:
  • Master of Science in Nursing or Doctor of Nursing Practice degree
  • National certification as a Nurse Practitioner
  • Active, unrestricted state license
  • Active DEA license
  • US work authorization
  • 2+ years of clinical experience as a Nurse or Nurse Practitioner
Preferred Qualifications:
  • Training or experience in geriatrics
  • 3+ years of clinical experience as a Nurse Practitioner
  • Experience with coding, ICD-10, and HCC
  • Experience working in a metrics-driven healthcare model
