Schedule: Monday to Friday; 8 AM – 5 PM

Job Title: Healthcare Innovations RN Specialist II

Location: Hybrid Work Environment

Contract Duration: 12+ months

Key Responsibilities:

  • Organize, coordinate, and deliver care coordination and case management services to members at high risk for health decline and adverse outcomes.
  • Manage both acute and chronic health conditions to enhance health and financial results through needs assessment and tailored interventions.
  • Employ a collaborative approach to evaluate, plan, implement, and monitor services necessary to address members' healthcare requirements.
  • Identify and leverage available resources to ensure quality and cost-effective healthcare outcomes through effective communication.
  • Ensure compliance with all relevant laws, regulations, and accreditation standards associated with the role.

NATURE AND SCOPE

  • This position does not involve direct management of personnel.
  • Reports to: Manager, Population Health Provider Liaison.
  • Essential Contacts: This role requires interaction with various stakeholders including Benefits Administration, Medical Directors, Underwriting teams, Group leaders, Delegated Vendors, Legal Departments, healthcare providers, and community resources.
