Job Overview

Position Summary:

The Clinical Care Coordination Assistant plays a vital role in collaborating with the Care Management team to provide essential clinical data for utilization assessments, ensuring the approval and certification of inpatient and post-discharge services.

Key Responsibilities:

  • Facilitates communication with Care Management, coordinating patient transfers and serving as a primary liaison among Care Management, admissions, and insurance providers.
  • Tracks and organizes communications, including important documentation such as IM letters, Denial Letters, and patient choice forms, while keeping the Care Management team informed.
  • Secures authorizations for patient admissions, accurately documenting information within the financial system, and collaborating with the Revenue Cycle team to guarantee precise inpatient encounters.
  • Oversees reviews and inquiries with payors, manages bed availability, and maintains an updated database of referral sources.
  • Handles multiple inquiries, presents referrals based on geographical location and service needs, and ensures timely follow-up on ongoing or pending inquiries.

Essential Skills:

Proficiency in electronic health record (EHR) systems, utilization management, case management, and acute care processes.

Preferred Qualifications:

  • Experience as a Medical Assistant is highly regarded.
  • Education: Associate degree in a relevant field is preferred.
  • Experience: A minimum of three years in a healthcare environment or one year in Utilization Management or Care Management within a medical group, acute care setting, or health plan is required.
  • Familiarity with Electronic Health Record (EHR) systems and Utilization Management applications, such as EPIC or 3M, is essential.
