Personal Care Assistant

The Certified Home Health Aide (CHHA) plays a crucial role within the Home Health Team, delivering direct patient support under the guidance of a case manager, and promptly reporting any alterations in the patient's condition.
Key Responsibilities:Patient Care Delivery
Assisting patients with personal grooming, including skin, oral, and hair care, as well as bathing.Helping patients with mobility, including getting in and out of bed and walking assistance.Supporting prescribed exercise routines as instructed by healthcare professionals.Preparing nutritious meals, including specialized diets, and aiding patients during mealtime.Assisting patients with restroom needs or using commodes, bedpans, or urinals.Performing household tasks that promote patient self-sufficiency at home and help avoid institutional care.Assisting patients with self-administered medications, while refraining from administering any medications directly.Carrying out additional tasks as directed by healthcare professionals for specific patients, which may include changing colostomy bags, non-sterile dressing changes, monitoring vital signs, and providing non-sterile hygiene care.Reporting any changes in the patient's condition and requirements to the supervising nurse or therapist.Maintaining accurate records of services rendered.Responding to emergency situations involving patients and promptly notifying the supervisor.Documentation and Reporting
Ensuring timely, clear, and comprehensive documentation of all care provided.Recording all communications with other healthcare providers.Notifying the case manager of any stated or observed changes in patient status.Submitting schedule adjustments and daily progress notes in accordance with agency policies.Engaging in care coordination meetings.Organization and Productivity
Effectively organizing, planning, prioritizing, and completing assignments within agency guidelines.Working independently while receiving supervision from the case manager every 14 days as necessary.Completing expense reports, time cards, and schedule logs accurately.Demonstrating efficient use of supplies.Professional Conduct
Exhibiting courteous and professional behavior towards patients and healthcare professionals while maintaining patient confidentiality.Participating in departmental quality assurance and utilization review activities as requested.Performing related duties as necessary.Supervisory Duties: This position does not include supervisory responsibilities.
Grade 165
Basic Qualifications: ExperienceSuccessful completion of a training program sanctioned by the Department of Health Services in accordance with applicable federal and state regulations. Must be open to working in a Labor Management Partnership environment.
Education
High School Diploma/GED; California Home Health Aide Certificate. Current BLS certification; California Driver's License. License, Certification, Registration Driver's License (California)
