Job Overview

We are seeking a skilled Care Coordinator to join our team at Thrive Behavioral Health. As a key member of our healthcare staff, you will work closely with clients, access teams, and community providers to provide essential support and coordination services. Your primary focus will be on alleviating mental health and substance use distress, eliminating access barriers to treatment, and providing necessary resources for our clients.

You will be responsible for researching and staying updated on additional resources and supports available to assist our clients. Additionally, you will establish ongoing relationships with community providers, agencies, and advocacy groups to build health equity and establish care coordination relationships that benefit our clients.
