The Senior Psychiatric Nursing Assistant IV (PNA Team Lead) is responsible for performing advanced rehabilitative patient care services under the clinical supervision of a registered nurse (RN), ensuring high-quality psychiatric nursing care in an inpatient environment.

This role involves coordinating emergency, health, and personal care services for patients, as well as guiding staff on hospital and program protocols.

As a vital member of the recovery or admissions team, you will provide verbal and supportive communication, documenting individual patient behaviors and progress towards their personalized recovery goals.

You will actively participate in shift reporting and team meetings, working with a degree of autonomy while exercising initiative and independent judgment.

Additional responsibilities may include participating in disaster response efforts and supporting the agency's continuity of operations.

Positions are available for programs serving adults, individuals with neurocognitive disabilities, geriatric patients, and those in a maximum-security unit.


Shifts include 7a-3p (Days), 3p-11p (Evening), and 11p-7a (Night).

Preference for program and shift will be considered during the selection process.

This position may offer additional compensation for evening, night, and weekend work.

Key Responsibilities:

Regular attendance is essential, and you may be required to work specific or rotating shifts, extended hours, or overtime as per agency policy.

As the Team Lead, you will guide other PNAs in maintaining therapeutic interactions with patients, fostering their living and social skills to encourage progress towards a less restrictive environment.

Utilize therapeutic communication techniques to effectively engage patients in achieving their recovery objectives.

If assigned to night shifts, ensure a calm and restful atmosphere to promote patient recovery.

In admissions, you will lead the team, assigning tasks such as taking vital signs and securing patient belongings while supporting patients through the evaluation process.

Monitor and assess patients for signs of illness, injury, or medication reactions that require intervention.

Coordinate care for patients of various age groups as directed by the registered nurse.

As the team lead, you will assign specific duties to other PNAs, including assisting patients with daily living activities to promote independence.

Instruct staff on escorting patients to therapeutic activities and appointments, ensuring adherence to hospital protocols.

Document patient goals, behaviors, and progress, contributing to the recovery team and ensuring all PNAs understand their roles in supporting treatment.

Develop treatment plans and guide PNAs in actively engaging patients to achieve their recovery goals, providing consistent support.

Assist licensed nurses in preparing patients for medication administration and treatment, including monitoring vital signs.

Perform duties of PNA I, II, and III as needed due to staffing shortages.

Utilize basic computer skills to communicate patient care information effectively and in a timely manner.

Promote safety through continuous monitoring and active engagement with patients to minimize risks.

Provide relevant feedback to supervising RNs regarding the performance of assigned PNAs to support comprehensive evaluations and identify training needs.


Facilitate programming on the treatment unit, co-facilitating groups as needed and encouraging patient participation.

Respond effectively to crisis situations, guiding PNAs in de-escalation techniques and treatment intervention strategies.

When necessary, follow RN directives to manage crisis situations, ensuring the safety of all individuals involved.

Communicate effectively with team members to maintain a culture of least restrictive interventions.

Perform CPR and use an Automatic External Defibrillator (AED) according to American Heart Association guidelines.

Instruct PNAs on providing timely, clinically relevant patient information to the RN to enhance safety and care quality.

Serve as a preceptor for new employees, including ongoing training for PNAs and other temporary staff.


Actively participate in shift reporting and serve on committees and performance improvement workgroups as assigned.

Model and encourage a commitment to safety and risk management strategies among subordinate staff.


Perform other duties as assigned, including participating in disaster response efforts as required.
Required Skills and Qualifications:

  • Proficient in CAPPS, CARE, Microsoft Word, and Excel.
  • Knowledge of patient aggression reduction strategies.
  • Understanding of patient care techniques and recovery programs.
  • Strong interpersonal and leadership skills.
  • Ability to operate computer and office equipment.
  • Competence in record-keeping and report preparation.
  • Experience in providing direction to others.
  • Proficient in English for effective communication.
  • Ability to provide patient care and administer first aid.
  • Willingness to collaborate and accept supervision.
  • Experience in training others.
