Job Summary

The Colorado Coalition for the Homeless is seeking a skilled Intake and Discharge Coordinator to facilitate the intake, discharge, and housing navigation of residents in Transitional Housing programs. This role requires strong organizational skills, attention to detail, and the ability to work collaboratively with various stakeholders.

Key Responsibilities
  • Act as the point person for daily operations of the Transitional Housing Intake and Discharge System, ensuring smooth coordination of daily intake processes.
  • Responsible for the fidelity of the reporting system and tracking tools, including master lists, HMIS, and EHR.
  • Utilize personal phone or provided work cell to field referrals from multiple points of entry.
  • Understand contractual requirements and work with the Program Manager to ensure Intake/Discharge Operations meet all targets.
  • Identify goals and strategies to better conduct integrated-care and/or VA intake procedures, better serving the needs of homeless individuals.
  • Collaborate with the Program Manager to communicate process changes in the Intake/Discharge arena to other CCH Programs, Partner Agencies, Funders, and community stakeholders.
  • Partner with the Program Manager to complete data delivery to the appropriate funding sources, such as City of Denver, MDHI, VA, and other investors and community partners.
  • Provide reports and updates to CCH Leadership as requested.
  • Provide ongoing housing case management and navigation services in conjunction with case management staff.
Requirements
  • Bachelor's degree or relevant experience in a related field required. Experience may be substituted.
  • Experience serving the homeless community or public health sector preferred.
  • Excellent organization skills and willingness to work flexibly under pressure.
  • Must have a valid driver's license.
