Job SummaryWe are seeking a highly skilled MDS Coordinator to join our team at The Springs at Wyandot Trail. As an MDS Coordinator, you will play a critical role in assessing the needs of our residents and developing personalized care plans to ensure their optimal health and well-being.
Key Responsibilities- Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create unique care plans based on findings.
- Promote cross-functional communication to ensure care plans are followed and residents receive seamless care.
- Ensure accurate and timely reimbursement for resident care services.
- Stay up-to-date with current federal, state, and local standards, guidelines, and regulations.
- Current, valid state RN license.
- At least 2 years of experience in MDS (resident assessment) and prior experience in Long-Term Care.
