Hospice Nurse Liaison Opportunity

St. Luke's Health System is seeking a dedicated and compassionate Registered Nurse (RN) to join our Hospice team as a Nurse Liaison. This role is a critical part of our post-acute care services, and we are looking for a skilled professional who can build strong relationships with hospital staff and providers to ensure seamless referrals and admissions for end-of-life patients.

Key Responsibilities:
  • Coordinate and facilitate referrals for end-of-life patients
  • Schedule initial assessments and admissions, establish patients' eligibility for care, and review referrals for appropriate placement
  • Coordinate discharge and post-discharge requirements, meet with patients/families to describe our services and educate them on the benefits of hospice care
  • Work closely with providers and staff for other possible referrals
Requirements:
  • ASN Degree
  • 1 year of RN experience, preferably in an Liaison role; Home Health, Acute Care, and/or Case Management experience helpful
  • Current RN License in the State of Practice; current Basic Life Support (BLS) Certification
  • Valid Driver's License and proof of Auto Insurance
