Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Liaison. In this role, you will play a critical part in building relationships with hospital staff and providers to ensure seamless referrals and admissions for end-of-life patients.

Key Responsibilities
  • Coordinate and facilitate referrals for end-of-life patients
  • Schedule initial assessments and admissions, and establish patients' eligibility for care
  • Review referrals for appropriate placement, coordinate discharge and post-discharge requirements, and meet with patients/families to describe our services and educate them on the benefits of hospice care
  • Develop and sustain strong working relationships with staff who refer to St. Luke's to increase referrals and admissions
  • Participate in marketing and education programs to increase awareness across the system
Requirements
  • ASN Degree
  • 1 year of RN experience, preferably in an Liaison role; Home Health, Acute Care, and/or Case Management experience helpful
  • Current RN License in the State of Practice; current Basic Life Support (BLS) Certification
  • Valid Driver's License and proof of Auto Insurance
