About the Role

We are seeking a skilled and compassionate Hospice Nurse Case Manager to join our team at Idaho Home Health and Hospice. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and support to individuals and their families at the end of life.

Key Responsibilities
  • Initial Nursing Evaluation: Conduct thorough assessments of patients to determine eligibility for hospice services within 48 hours of referral.
  • Plan of Care Development: Collaborate with patients, families, and healthcare professionals to create personalized plans of care that address physical, psychosocial, and environmental needs.
  • Continuous Assessment and Evaluation: Regularly reassess patients' conditions, identifying areas for improvement and making necessary adjustments to ensure optimal care delivery.
  • Documentation and Communication: Maintain accurate and detailed clinical notes, documenting patient progress, goals, interventions, and responses to care.
  • Interdisciplinary Collaboration: Work closely with patients, families, attending physicians, and other healthcare professionals to provide comprehensive care and support.
Requirements
  • Experience: One year of clinical experience as a Registered Nurse.
  • Licensure: Current RN licensure in the state of practice and one year of clinical experience.
  • Certifications: Current CPR certification and valid driver's license with vehicle insurance and access to a dependable vehicle or public transportation.
