We are seeking a skilled and compassionate Hospice Nurse Liaison to join our team at St. Luke's Health System. As a key member of our post-acute care team, you will play a critical role in coordinating and facilitating referrals for end-of-life patients.
Key Responsibilities- Coordinate and facilitate referrals for end-of-life patients
- Schedule initial assessments and admissions
- Establish patients' eligibility for care
- Review referrals for appropriate placement
- Coordinate discharge and post-discharge requirements
- Meet with patients/families to describe our services and educate them on the benefits of hospice care
- Work closely with providers and staff for other possible referrals
- ASN Degree
- 1 year of RN experience, preferably in an Liaison role; Home Health, Acute Care and/or Case Management experience helpful
- Current RN License in the State of Practice
- Current Basic Life Support (BLS) Certification
- Valid Driver's License and proof of Auto Insurance
- Degree advancement through tuition reimbursement up to $5,250 per calendar year
- Certification reimbursement program for achieving certifications/credentials that above and beyond requirements in your role AND earn extra pay for eligible certifications
