- Assess and Educate: Conduct thorough assessments of patients and their families, providing education on home safety and end-of-life care.
- Communicate and Coordinate: Effectively communicate with patients, families, and healthcare providers to ensure seamless care coordination.
- Document and Report: Maintain accurate and detailed records of patient care, including assessments, interventions, and outcomes.
- Advocate for Patients: Serve as a patient advocate, ensuring that their needs and preferences are respected and prioritized.
- Education and Licensure: Current RN licensure in the state of employment and a minimum of one year of nursing experience, preferably in a Home Health/Hospice setting.
- Certifications and Skills: Certification in Hospice and Palliative Nursing is a plus, but not required. Strong clinical, organizational, and interpersonal skills are essential.
- Physical Demands: Ability to lift up to 20 pounds and occasional travel required.
