Job Description

Arbor Care Solutions is seeking a highly skilled and dedicated Community Support Team Leader to join our team.

Job Summary: The Community Support Team Leader will provide clinical and administrative supervision to the Community Support Team, ensuring the delivery of high-quality mental health and substance abuse rehabilitation services to adults.

Key Responsibilities:

  • Supervise and coordinate the activities of the Community Support Team, ensuring compliance with agency policies and procedures.
  • Provide clinical supervision to team members, including case management, treatment planning, and crisis intervention.
  • Coordinate the movement of consumers across levels of care, ensuring seamless transitions and continuity of services.
  • Develop and implement Person-Centered Plans, ensuring consumer-centered and trauma-informed care.
  • Monitor and evaluate the effectiveness of services, making recommendations for improvement.
  • Collaborate with internal and external stakeholders, including DSS, DJJ, and LME, to ensure compliance with regulatory requirements.
  • Participate in quality assurance and improvement activities, ensuring the delivery of high-quality services.

Requirements:

  • Master's degree in a related human service field or equivalent experience.
  • Minimum of 2 years of experience in mental health and substance abuse rehabilitation services.
  • Valid licensure or certification in a relevant field.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
