AP-Community Support Specialist

Arbor Care Solutions is seeking a highly skilled AP-Community Support Team Professional to join our team. As a key member of our Community Support Team, you will play a vital role in providing mental health and substance abuse rehabilitation interventions to adults (age 18 and older) in achieving rehabilitative and recovery goals.
Key Responsibilities:
- Assist with therapeutic interventions to rehabilitate functional skills, adaptation, socialization, relational, and coping skills, self-management of symptoms, daily and community living skills, and behavior and anger management skills.
- Assume the role of advocate, broker, coordinator, and monitor of service delivery system on behalf of the consumer.
- Coordinate movement across levels of care, including discharge planning and community re-entry following hospitalization, residential services, and other levels of care.
- Provide therapeutic mentoring that directly increases the acquisition of skills needed to accomplish the goals of the Person Centered Plan.
- Psycho-educate and train family, unpaid caregivers, and others who have a legitimate role in addressing the needs identified in the Person Centered Plan.
- Direct preventive and therapeutic interventions that will assist with skill building, relapse prevention, and disease management strategies.
- Ongoing symptom monitoring and management, including medication monitoring with reports to medical providers.
- Service coordination activities within the established Person Centered Plan, including input into Person Centered Plan modifications.
- Ensure compliance to all applicable rules, regulations, and policy and procedures of the agency and governing standards.
- Participate in policy and program implementation, assist with meeting deadlines, and work with the mental health agency to solve day-to-day problems regarding clients and/or their families.
- Audit clinical charts to ensure completeness, accuracy, and regulatory compliance.
- Provide On-Call support under the supervision of the QP and maintain ongoing communication with agency stakeholders such as DSS, DJJ, and LME.
- Ensure all paperwork is submitted in a timely manner and participate in quality assurance and improvement programs.
- Participate in treatment team meetings along with other team members.
Requirements:
- Graduate of a college or university with an advanced degree in a related human service field and having less than 1 year of full-time, post-graduate accumulated MH/DD/SAS experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
- Graduate of a college or university with a Baccalaureate degree in a human service field and having less than 2 years full-time post-bachelor's degree accumulated MH/DD/SAS experience with the population service, or a substance abuse Professional with less than 2 years experience of full-time, post-bachelor's degree supervised experience in alcoholism and drug abuse counseling.
- Graduate of a college or university with a Baccalaureate degree in a non-related field and having 4 years of full-time, post-bachelor's degree MH/DD/SAS experience with the population serviced, or a substance abuse professional who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
- Graduate of a college or university with a bachelor's degree in a field other than human services with less than four years of full-time post-bachelors degree accumulated MH/DD/SAS experience with the population served, or a substance abuse professional with less than four years of full-time post bachelor's degree accumulated experience in alcoholism and drug abuse counseling.
- A registered Nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with less than four years of full-time accumulated experience in MH/DD/SAS with the population served.
