Aveanna Healthcare LLC is seeking a skilled Physical Therapy Assistant to join our team in providing high-quality care to patients in their homes.
Key Responsibilities- Administer physical therapy treatments to patients in accordance with the Plan of Care developed by the Physician and Physical Therapist.
- Evaluate and document the effectiveness of physical therapy services and the patient's response to treatment.
- Assess the home environment to identify safety hazards and recommend changes to eliminate or reduce these hazards.
- Recognize and identify the need for initiation of services from other disciplines or resources and document these needs.
- Assess social, emotional, and physical factors within the patient's home that affect patient care and response to treatment.
- Attend interdisciplinary team meetings and identify problems related to patient's response to treatment.
- Report any adverse changes in the patient's condition to the supervising Physical Therapist and/or Care Manager.
- Current license as a Physical Therapy Assistant in the State of GA.
- Graduate of an accredited School for Physical Therapy Assistants.
- Preferably one year of clinical experience in a home health care setting.
