As a key member of our healthcare team, the successful candidate will be responsible for collecting and assessing resident information, ensuring compliance with federal and state regulations, and collaborating with interdisciplinary colleagues to provide high-quality care.

Key Responsibilities
  • Collect and assess resident information for the Minimum Data Set (MDS) process, ensuring timely completion and accuracy of assessments.
  • Collaborate with residents, families, and healthcare teams to develop and implement individualized care plans.
  • Utilize state and federal guidelines to ensure documentation meets necessary guidelines and reimbursement standards.
  • Review and validate MDS responses from all disciplines, ensuring accuracy and completeness.
  • Work with the Health Information Management (HIM) Director and Quality Care Coordinator to conduct concurrent MDS reviews and ensure achievement of maximum allowable RUG categories.
  • Assist with completing nursing assessments and developing written plans of care for each resident.
  • Ensure legal compliance for the facility by following the MDS process.
  • Assist with determining coverage for skilled nursing services and provide complete documentation to prevent payment denials.
  • Utilize the RAI, MDS 3.0, QM, and QRP manuals to ensure the highest quality of care is provided, documentation supports outcomes, and maximum reimbursement is achieved.
Requirements
  • Current NYS Licensed Registered Nurse.
  • Knowledge of MDS 3.0, CAAs, Medicare, and Medicaid guidelines for coverage and reimbursement.
  • Excellent written and oral communication skills, with the ability to work independently and under time constraints.
  • Long-term care experience is helpful but not necessary.
  • Basic computer skills.
