About the Role

Hospice of Santa Cruz County is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Hospice Nurse. As a member of our care team, you will provide bedside care to our patients and their families, working in partnership with our interdisciplinary team to deliver end-of-life support and guidance.

Key Responsibilities
  • Provide high-quality, patient-centered care to individuals with serious illnesses in their homes or facilities
  • Collaborate with the HSCC Care Team to develop and implement individualized care plans
  • Offer emotional support and guidance to patients and their families during the end-of-life journey
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Participate in ongoing education and training to stay current on best practices in hospice care
