Job Summary:

The Hospice and Palliative Care Nurse Practitioner plays a vital role in providing high-quality patient care in various settings. Collaborating with the Medical Director, Chief Clinical Officer, and Hospice and Palliative Teams, this professional assesses patient needs, develops care plans, and coordinates services to ensure seamless transitions in care.

Responsibilities:

  • Conducts comprehensive patient evaluations, including bedside and telehealth assessments, to identify patient needs and develop personalized care plans.
  • Coordinates with RNCM, Hospice Physicians, and Primary Care Providers to ensure timely and effective care transitions, including medication reconciliation and family communication.
  • Provides Advanced Care Planning and Goals of Care conversations to patients and families, ensuring their values and preferences are respected.
  • Orders, performs, and interprets laboratory and radiology tests in Hospice settings, ensuring accurate diagnoses and effective treatment plans.

Qualifications:

  • Current Nurse Practitioner License for California.
  • Minimum 1-year Nurse Practitioner experience preferred.
  • Current DEA license and CPR Certification.
  • Previous Hospice and/or Palliative Care experience is highly valued.
  • Active Driver's license in California.
  • Graduate from NLN accredited school of Nursing.
  • Master's degree in appropriate field of nursing.

Working Environment:

  • Must possess mobility to work in a standard office setting and to use standard office equipment, including a computer.
  • Makes patient visits in SNFs, RCFEs, and Home settings.
  • Potential travel in inclement weather.
