Job Summary

We're seeking a skilled Hospice Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you'll be responsible for performing Face-to-Face (F2F) encounters, gathering clinical information, and communicating findings to our hospice physician. Your expertise will play a critical role in supporting patient care and eligibility.

Responsibilities
  • Conduct thorough physical assessments for F2F encounters within regulatory time frames
  • Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists to the physician
  • Make recommendations to the IDG based on F2F assessment findings
  • Notify the hospice agency when unable to complete a scheduled F2F visit
Requirements
  • Master's or higher degree in Nursing
  • Graduate-level nurse practitioner education program accreditation
  • Current state license as a registered nurse
  • Current state license or certificate as an advanced practice nurse
  • Current national certification as a Nurse Practitioner (ANCC or AANP)
  • Collaborative agreement in place

Compensation may vary within the salary range provided based on several factors, including location, experience, education, skills, licensure, certifications, and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range – $80.00 - $95.00
