Hospice and Palliative Care Nurse Practitioner Job Summary
The Hospice and Palliative Care Nurse Practitioner will work collaboratively with the Medical Director, Chief Clinical Officer, and Hospice and Palliative Teams to provide high-quality patient care services in various settings. This role requires strong clinical skills, excellent communication, and a passion for delivering compassionate care to patients and their families.
Key Responsibilities:
- Conduct thorough patient evaluations, including physical examinations, medical histories, and laboratory interpretations.
- Develop and implement individualized care plans, ensuring timely transitions in care and effective pain and symptom management.
- Collaborate with interdisciplinary teams, including social workers, nurse case managers, and ancillary staff, to coordinate hospice and palliative care services.
- Provide education and support to patients, families, and caregivers on advanced care planning, goals of care, and end-of-life care.
- Order, perform, and interpret laboratory and radiology tests as needed.
Requirements:
- Current Nurse Practitioner License for California.
- Minimum 1-year Nurse Practitioner experience preferred.
- Current DEA license and CPR Certification.
- Previous Hospice and/or Palliative Care experience a plus.
- Active Driver's license in California.
- Graduate from NLN accredited school of Nursing.
- Master's degree in appropriate field of nursing.
Working Environment:
- Must possess mobility to work in a standard office setting and to use standard office equipment, including a computer.
- Makes patient visits in SNFs, RCFEs, and Home settings.
- Potential travel in inclement weather.
