Job Description

**Job Summary**

We are seeking a highly skilled Patient Care Coordinator to join our team at Alternatives in Treatment LLC. The successful candidate will be responsible for providing exceptional patient care and support, ensuring a seamless and efficient experience for our patients.

Key Responsibilities:

  • Provide ongoing professional, technical, and clinical expertise to patients and their families.
  • Coordinate care and individual patient needs to enhance the quality of patient management, treatment, and satisfaction.
  • Orientate new patients to facility policies and procedures, introducing them to clinical staff and program rules.
  • Assist patients in navigating facility resources, including medication times, catering requests, and cigarette orders.
  • Troubleshoot and collaborate with the clinical team to investigate patient concerns and determine the appropriate response.
  • Create healthy boundaries and motivate group attendance within the treatment milieu.
  • Enhance the therapeutic environment by being a point of contact and support system to departments.
  • Assess data from patient relation issues to identify areas of concern and recommend proactive solutions.
  • Play a key role in maintaining a high level of patient satisfaction by providing exceptional customer service and addressing patient concerns.
  • Assist the case management team with meeting patient legal obligations, such as court dates and community service.
  • Work collaboratively with the clinical treatment team to guide, assist, and motivate patients to achieve their goals.
  • Assist with de-escalation and intervention of behaviors/AMA Risks.
  • Support the PCA team with observation of rounds, necessary random searches, intakes, discharges, and other duties as assigned.
  • Collaborate with the clinical team to evaluate and identify peer leadership within the community.

Qualifications:

  • High School Diploma or GED equivalent with a combination of education and work experience.
  • Must comply with the minimum DCF requirements for staff with patient contact.
  • Knowledgeable in the field of Substance Abuse treatment.
  • Relevant computer software and hardware applications proficiency, including Word, Excel, PowerPoint, Outlook, Electronic Medical Records, and Billing Systems.
