As a vital member of a dedicated team, an Inspector V plays a crucial role in safeguarding the health and safety of residents by performing thorough inspections and assessments in long-term care facilities across Texas. This includes licensed nursing homes, assisted living establishments, day activity and health service sites, and pediatric extended care centers, ensuring compliance with both state and federal regulations.

In this dynamic field role, the Inspector V investigates complaints and incidents, addressing concerns related to abuse, neglect, or exploitation, either independently or as part of a team.

The inspector operates primarily in the field, with minimal office duties, although the position requires the preparation of detailed reports outlining any identified deficiencies or violations, suggested corrective actions, and narratives to support regulatory conclusions.

Additionally, the inspector is tasked with drafting comprehensive reports that back up recommended actions and participating in surveys, inspections, and investigations alongside assigned teams, accepting assignments as necessary.

Key Responsibilities:

Under the guidance of a program manager, the Inspector V conducts on-site inspections for licensure, investigates complaints and incidents, and performs annual comprehensive surveys to assess compliance with health and safety regulations.

Effectively communicates with internal and external stakeholders to provide, exchange, or verify information, address inquiries, and resolve issues related to services provided in Texas long-term care facilities, ensuring that services meet the specialized needs of residents.


Conducts exit conferences with facility personnel to discuss findings and may assist in formulating plans to rectify deficiencies.

Completes inspection or survey reports that detail deficiencies, recommended corrective measures, and narratives supporting regulatory findings.

Visits long-term care facilities to confirm the rectification of previously identified regulatory violations.
Performs quality assurance reviews of documents prepared by others, such as publications, assessments, and reports.

Occasionally, may be required to provide testimony at hearings or court proceedings regarding policies, procedures, findings, or activities.

Completes referrals for actions as appropriate.
