About the RoleThis is a challenging and rewarding opportunity to join our Vascular Surgery department as a Registered Professional Nurse. In this role, you will be responsible for assessing, coordinating, planning, directing, implementing, and managing the needs of assigned patients throughout their episode of care. Your focus will be on progress toward discharge, including during transitions within the acute care stay.
As a key member of the caregiving team, you will ensure that competent and compassionate patient care is uniformly provided to our customers, including referring facilities, hospital staff, and patients in various settings. You will also collaborate with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
In addition to your clinical expertise, you will possess excellent communication and interpersonal skills, enabling you to effectively interact with patients, families, and colleagues. Your ability to work collaboratively and provide empathetic support will make a significant difference in the lives of our patients and their loved ones.
Responsibilities- Assess and coordinate patient care needs, prioritizing tasks and resources to optimize outcomes.
- Develop and implement individualized care plans, taking into account patient preferences, goals, and medical history.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and effective communication.
- Maintain accurate and up-to-date records of patient information, including assessment data, treatment plans, and progress notes.
- Participate in quality improvement initiatives, contributing to the development of best practices and evidence-based protocols.
To be successful in this role, you will need:
- A current RN license and active registration in New York State.
- Experience working in an acute care setting, preferably in vascular surgery or a related field.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
- A commitment to providing compassionate and patient-centered care, with a focus on empathy and understanding.
- The ability to work collaboratively as part of a high-performing team, with a flexible and adaptable approach to changing priorities and circumstances.
Albany Medical Center offers a competitive salary range of $83,000 - $124,000 per year, depending on experience. Our comprehensive benefits package includes:
