Job Overview

We are seeking skilled Nurse Practitioners or Physician Assistants to join our team for PRN/Part Time - Home Risk Assessments. As a valued member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, as well as provide health education and referrals.

Responsibilities
  • Conduct comprehensive health assessments in a variety of venues, including home visits and telehealth visits
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care delivery
  • Provide patient education and follow-up, as needed
Requirements
  • Master's degree required; OR commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP/PA licensure in state of practice, including prescription authority
  • Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner/Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Possess excellent verbal and written communication skills

Travel Requirements
