Medical Professional for Home Risk Assessments

We are seeking a skilled Medical Professional to join our team at Matrix Medical Network. As a Nurse Practitioner or Physician Assistant, you will be responsible for conducting comprehensive health assessments in the comfort of patients' homes.
Job Responsibilities- Conduct thorough assessments of adult and geriatric patients, including medical history, diagnosis, and treatment.
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to provide high-quality patient care.
- Provide education to patients and their families on healthy lifestyle choices and disease management.
- Master's degree in Nursing or Medicine required, with commensurate experience and licensure.
- Current RN and NP or PA licensure in the state of practice, with prescription authority or ability to obtain.
- Board certification in a Matrix-approved specialty is preferred.
- A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant is required.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
