Position Overview
The Patient Flow Coordinator RN plays a critical role in facilitating the admission transfer and bed planning process for patients within the WakeMed System. This position works closely with healthcare providers at outlying facilities to arrange transfers of patients into the WakeMed system, ensuring a seamless transition and optimal bed planning for all unscheduled admissions.
Key Responsibilities
- Collaborate with healthcare providers, emergency department staff, OR/PACU teams, inpatient units, nursing leaders, and clinical administrators to ensure efficient patient flow and bed planning throughout the system.
- Ensure compliance with federal, state, and third-party utilization management regulatory requirements.
- Provide direct coordination of internal patient flow and external transfers through comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
Requirements
- 5 years of experience in acute care nursing, with a focus on utilization management and/or critical care (ICU/CCU) preferred.
- Bachelor's degree in nursing required.
- Registered Nurse licensure required, with national certification in nursing required within 1 year of hire.
- Night shift work, with every-other-weekend requirements and on-call responsibilities.
