Job SummaryThe Patient Flow Coordinator RN plays a critical role in facilitating the admission transfer and bed planning process for patients within the WakeMed System. This position works closely with providers at outlying facilities to arrange transfers of patients into the WakeMed system and ensures a transfer plan for the appropriate bed type level of care and status is made for all unscheduled admissions.
Key Responsibilities- Collaborate with providers, emergency department, OR/PACU, inpatient units, nursing leaders, and clinical administrators to ensure appropriate patient flow and bed planning throughout the system.
- Ensure compliance with federal, state, and third-party utilization management regulatory requirements.
- Work with the Capacity and Transfer Management Center to coordinate internal patient flow and external transfers through comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
- 5 years of nursing experience in acute care required.
- Experience in utilization management and/or critical care (ICU/CCU) preferred.
- Bachelor's degree in nursing required.
- Registered nurse licensure required.
- National certification in nursing required within 1 year of hire.
7pm-7am, every other weekend, with call requirements.
