Key Responsibilities:
- Engage with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies regarding the care requirements of all residents.
- Facilitate incidental medical, dental, vision, hearing, and podiatry services for residents.
- Execute nursing tasks as directed and in accordance with the Nurse Practice Act, including administering medications and performing treatments as prescribed by a physician.
- Support Medication Technicians to ensure the proficient delivery of medications to residents.
- Assist with resident assessments as instructed by the Health Services Director.
- Conduct ongoing staff evaluations and monitor residents to ensure that community staff is updated on changes in residents' physical and emotional well-being. Communicate relevant resident information to physicians and other healthcare professionals, alerting the Executive Director and responsible parties to any concerns as necessary.
- Maintain comprehensive documentation in resident charts consistently, adhering to company policies and state regulations.
- Review and supervise staff utilization of the Shift Report daily and participate in change of shift meetings to relay pertinent information regarding resident care needs.
- Coordinate prescription orders with physicians' offices and oversee pharmacy medication delivery. Manage the central storage and distribution of medications.
- Communicate with residents and their families regarding the community's preferred pharmacy and incontinence supply programs.
- Must possess a valid California nursing license as a Licensed Vocational Nurse or Registered Nurse in good standing.
- Preferred: At least one (1) year of experience in a Residential Care Facility for the Elderly (RCFE).
- Proficient in operating a personal computer and familiar with MS Office applications (Word, Excel, etc.).
- Able to use standard office equipment, including fax machines, copiers, and telephones.
- Knowledgeable about or willing to learn the principles and practices of assisted living and dementia care.
- Capable of working with seniors and interacting patiently with cognitively impaired individuals.
- Must obtain and maintain valid first aid and CPR certification.
- Must successfully pass a Criminal Background check and Health Screening tests, including physical and TB tests.
- For the health and safety of our team members and residents, Oakmont Management Group may require team members to be vaccinated, participate in daily screenings, surveillance testing, and wear face coverings and other personal protective equipment (PPE) to prevent the spread of COVID-19 or other communicable diseases, in accordance with regulatory guidelines.
