Weekend Hospice Care Nurse Specialist

Suncrest Home Health & Hospice is seeking a dedicated Weekend Runner (RN) to join our team. As a Weekend RN, you will be responsible for providing exceptional care and services to our patients on Saturdays and Sundays, from 9am to 9pm.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.
This role involves working in either a Case Management, Admissions, or Resource role, requiring strong communication and collaboration skills. You will work closely with patients on a one-on-one level and receive support from a patient care team.
Requirements- Current Registered Nurse (RN) licensure in good standing
- Ability to work independently and as part of a team
- Excellent time management and prioritization skills
- Valid driver's license and reliable transportation
