HIV Medical Case Manager and Data Specialist

Job Summary:
The Commonwealth of Virginia is seeking a highly skilled HIV Medical Case Manager and Data Specialist to join our team. As a key member of our healthcare delivery system, you will play a vital role in providing coordination, guidance, and assistance to individuals living with HIV/AIDS.
About the Role:
This is a challenging and rewarding position that requires a strong understanding of case management principles, excellent communication skills, and the ability to work effectively with diverse populations.
Responsibilities:
- Medical Case Management: Provide comprehensive case management services to HIV-positive clients, including assessments, care planning, and coordination of medical and support services.
- Data Entry: Collect and enter data into the CAREWare data system, ensuring accurate and timely reporting.
- Quality Management: Participate in quality improvement activities to ensure the highest level of service delivery.
Requirements:
- A minimum of 3 years of experience in case management, preferably in a healthcare setting.
- Knowledge of Medicare, Medicaid, and other state or federal funded programs for eligible individuals.
- Knowledge of HIPAA and ability to implement privacy and confidentiality practices.
- Excellent customer service and interpersonal skills.
- Ability to work independently and as part of a team.
Preferred Qualifications:
- Knowledge and experience with the Ryan White grant program.
Salary:
$65,000 - $85,000 per year, depending on experience.
