Position Overview

The Care Coordinator plays a crucial role within the interdisciplinary Care Management team, focusing on ensuring seamless patient care for individuals identified as high-risk. Under the supervision of Nurse and Social Work Care Managers, the Care Coordinator will support at-risk patients through various means aimed at enhancing their overall health outcomes. Responsibilities include conducting telephonic outreach to engage patients in Care Management programs and facilitating collaboration between healthcare providers and community resources.

Key Responsibilities (including but not limited to):

  • Identify and address barriers to patient care
  • Enhance communication between patients and primary care providers
  • Collaborate daily with the Care Management Team
  • Act as an advocate for patients
  • Perform additional assigned tasks as needed
  • Manage inbound calls to the Care Management queue
  • Conduct outbound engagement calls as necessary
  • Prepare patient records for outreach initiatives
  • Determine patient eligibility for Care Management programs
  • Utilize various systems to collect relevant information
  • Document all interactions accurately in the Electronic Medical Record (EMR)

Qualifications and Experience

  • High school diploma or GED
  • A minimum of two (2) years of experience in settings such as Acute Care, Ambulatory Care, Specialty Care, Physician Offices, Care Management, or Home Health Care
  • Ability to travel for training and meetings as required

Skills and Competencies

  • Ability to multitask and prioritize in a fast-paced environment
  • Knowledge of HIPAA regulations and confidentiality standards
  • Strong interpersonal and organizational abilities
  • Compassionate, patient-centered approach
  • Adaptability to new technologies and software
  • Proficiency in EMR systems, Outlook, and basic Excel

*This position offers a hybrid-remote work arrangement, and candidates must reside in the designated area to be considered.

We provide a comprehensive benefits package, including health and dental insurance, 401K, vacation, and paid time off, along with a supportive work environment.

Compensation ranges from $18.00 to $24.00 per hour, with individual salaries determined based on various factors such as market data, education, experience, qualifications, and internal equity considerations.
