Summary

Marion County is seeking a highly skilled Exceptional Needs Care Coordinator to join our Adult Behavioral Health team. As a critical member of our multidisciplinary treatment team, you will provide services to individuals with serious and persistent mental illnesses and complex needs, many of whom also have co-occurring substance use disorders and medical conditions.

Key Responsibilities
  • Exceptional Needs Care Coordination: Act as a liaison between ABH and various stakeholders, including Oregon State Hospital, acute care hospitals, and coordinated care organizations (CCO). Collaborate with individuals, partners, and stakeholders to assess needs, determine eligibility, and authorize services.
  • Licensed Care/Residential Coordination: Assess individuals' needs, determine eligibility for services, and authorize licensed care/residential placements. Identify and coordinate placements in appropriate licensed care settings and negotiate with Oregon Health Authority for Rate Reviews.
  • Assessment & Treatment Planning: Complete intake and annual comprehensive mental health assessments, develop individualized treatment plans, and direct provision of treatment services. Meet performance expectations for time and billing standards.
Requirements
  • Master's degree in psychology, social work, recreational therapy, music therapy, art therapy, or a behavioral science field; OR Bachelor's degree in nursing, or occupational therapy and licensed by the State of Oregon.
  • One year of postgraduate degree experience in the behavioral health field.
  • Must be licensed by the Oregon Board of Licensed Professional Counselors, licensed by the Oregon Board of Licensed Social Workers, or credentialed as a Qualified Mental Health Professional (QMHP) within the first 30 days of employment.
