As a Nurse Care Coordinator, you will be responsible for providing high-quality nursing care to individuals with developmental disabilities. Your primary focus will be on coordinating care, monitoring health needs, and delegating tasks to unlicensed staff.
- Key Responsibilities:
- Provide nursing care, including administering medications and personal care activities.
- Coordinate care for customers with aging and increased medical needs.
- Monitor health care needs and delegate tasks to unlicensed staff.
- Periodically observe and evaluate staff skills and abilities.
Requirements:
- Two years' experience in developmental disabilities desired.
- Current license as a Registered Nurse in Oregon with no restrictions.
- High School Diploma/GED.
- CPR and First Aid certification.
Preferred Qualifications:
- Knowledge of RN nursing practice and skills.
- CPR and First Aid Skills.
- Knowledge of OAR applicable standards and other state requirements.
Benefits:
- 4 weeks Accrued Paid Time Off (PTO) per year.

- 50% Medical coverage for dependent/ spouse.
Supervisory Responsibility:
Oversees LPN's delivery of nursing care.
Essential Position Functions and Key Work Processes:
- Provide nursing care, such as administering medications, personal care activities, g-tube feedings etc. as required for customer care.
- Coordinate care for customers who are aging and have increased medical needs.
- Monitor the health care needs of customers with nurse/patient relationships.
