Job Description

Fresenius Medical Care North America is seeking a highly skilled Medical Social Worker - Quality of Life Specialist to join our team.

About the Role

This is an entry-level MSW role that focuses on supporting patients with end-stage renal disease (ESRD) and their families in navigating the healthcare system. The ideal candidate will have excellent communication skills, empathy, and the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Clinical Services:
    • Conduct patient assessments to identify psychosocial needs and develop personalized care plans.
    • Provide education and support to patients and their families on kidney disease, treatment options, and lifestyle changes.
    • Collaborate with the interdisciplinary team to ensure comprehensive care and address non-adherence and quality of life concerns.
  • Customer Service:
    • Deliver exceptional customer service to patients, families, and healthcare professionals.
    • Address patient concerns and issues in a timely and professional manner.
  • Documentation and Reporting:
    • Maintain accurate and detailed records of patient interactions and interventions.
    • Prepare reports and presentations for stakeholders as required.
  • Education and Training:
    • Develop and deliver training programs for staff on psychosocial topics related to ESRD.
    • Stay up-to-date with industry developments and best practices in social work and patient care.
Requirements:

We are looking for a motivated and compassionate individual who possesses the following qualifications:

  • Bachelor's degree in Social Work (BSW) or Master's degree in Social Work (MSW)
  • State-specific licensure required
  • 0-2 years' experience in medical social work or a related field
