Patient Care Social Worker

We are seeking a skilled Patient Care Social Worker to join our team at Lincoln Community Health Center, Inc.
Job SummaryThe ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, staff, and community resources. A Master's degree in social work is required, along with a minimum of three years of post-master's work experience as a clinical social worker in a health or mental health setting.
Key Responsibilities:- Provide initial assessment, diagnosis, evaluation, and case management services for adult patients, with screening for adolescents and children.
- Develop and implement health promotion and disease prevention programs and therapeutic services for individuals, groups, and families.
- Serve as a resource person or coordinator for designated unit functions.
- Master's degree in social work (MSW) required.
- Three years of post-master's work experience as a clinical social worker in a health or mental health setting, or five years' experience in a non-clinical setting.
- N.C. LCSW licensure required.
