Job Summary:

The Workforce Case Manager will serve as a bridge to ongoing services, an advocate for guest needs, and an assistant to the removal of barriers to progress for guests in our dining rooms. This role requires excellent communication, organization skills, and the ability to interact with guests, staff, and volunteers.

Key Responsibilities:
  • Ensure accurate electronic records of guest encounters are generated within 24 hours of interaction using the HMIS database
  • Obtain intake demographic and current needs information from dining room guests
  • Monitor guest care plans, goals, and service referrals to ensure engagement
  • Work with volunteers and dining staff to obtain guest information and needs
  • Engage team members in joint problem-solving
  • Connect to and manage established relationships with community partners and employers who assist our guests to find and retain employment
  • Provide supervision, support, direction, and engagement to Workforce Opportunities Fellows
  • Establish HMIS data entry and documentation standards for dining room guests
  • Offer help to team members when able; ask for help when needed
  • Share ideas on how the dining room team can continue to improve and provide the best possible customer service to our volunteers and program partners
  • Model skills and assist team members to verbally de-escalate guests as needed
  • Coordinate incident intervention with other staff and security when needed
  • Appropriately document and inform leadership of any incidents or concerns
  • Attend staff meetings, trainings, and agency events as requested or required
Requirements:
  • Excellent communication, organization skills, and ability to interact with guests, staff, and volunteers required
  • Related experience working with vulnerable populations, volunteers, and staff in a temporary shelter environment preferred
  • Knowledge of available community support and services is beneficial and will be shown preference
  • Strong computer skill, proficiency with Microsoft Suite programs, experience in HMIS, and excellent electronic communication and documentation abilities preferred
  • This position may require transportation of clients to connect with services. 39-month motor vehicle record will be required
  • Ability to report to more than one work site is required
