About the Role

We are seeking a compassionate and skilled Hospice Admissions Nurse to join our team at Assured Home Health & Hospice. As a key member of our interdisciplinary team, you will play a vital role in ensuring that patients and their families receive the highest level of care and support.

Responsibilities
  • Execute the hospice admission process in compliance with federal and state regulations
  • Advocate for patients and families to meet their stated needs and refer them to other resources as appropriate
  • Develop trusting relationships with patients and families as an end-of-life care expert
  • Maintain confidentiality and safety of patient/client clinical records
  • Communicate effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding new admissions
Requirements
  • Licensed registered nurse in the state of operation
  • At least 2 years of experience in a hospice, home health, or other healthcare service delivery system setting
  • Current CPR certification
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle or public transportation
