After-Hours Hospice Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team as an After-Hours Registered Nurse. As a member of our hospice care team, you will provide end-of-life care to patients and their families after regular business hours, including evenings and weekends.
Key Responsibilities:- Triage patient needs over the phone and provide timely delivery of hospice care.
- Respond to crisis calls from patients and families with urgency and empathy.
- Conduct initial nursing evaluations to determine eligibility for hospice services.
- Perform after-hours visits, including admission visits, death visits, recertification visits, and routine visits.
- Assess patient and family needs, identify problems, and develop goals and interventions.
- One year of clinical experience as a Registered Nurse.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
