Afterhours Registered Nurse Hospice Specialist

We are seeking a dedicated and compassionate Afterhours Registered Nurse to join our team at Assured Hospice of Port Angeles, a part of LHC Group. As a valued member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the Role:
- Provide comprehensive and timely delivery of hospice care to patients and their families after regular business hours, including nights and weekends.
- Triage patient needs over the phone until a nurse can provide an in-person visit.
- Respond to crisis calls from patients and families afterhours with urgency and compassion.
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Perform afterhours visits, including admission visits, death visits, recertification visits, and routine visits.
- Identify and address the physical, psychosocial, emotional, and environmental needs of patients and their families.
Requirements:
- One year of clinical experience.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
