Job Title: Hospice Nurse Practitioner

We are seeking a compassionate and skilled Hospice Nurse Practitioner to join our team at Assured Home Health & Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of palliative care.

Job Summary:

The Hospice Nurse Practitioner will be responsible for providing consultation in palliative care, symptom management, and supportive care to meet the needs of our Hospice patients. This includes making face-to-face encounter visits, reporting clinical findings, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:
  • Provide consultation in palliative care, symptom management, and supportive care to Hospice patients
  • Make face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and report clinical findings to the Hospice Medical Director
  • Consult with the Hospice Medical Director as needed, inform primary physician of recommended services, and collaborate with other physicians as needed
  • Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family response to hospice care
  • Instruct and supervise patients and families in self-care techniques when appropriate
  • Maintain accurate and relevant clinical notes regarding patient condition and report patient condition and any identified needs/issues to Administrator and Hospice Medical Director
Requirements:
  • Minimum of five years nursing experience, with clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred
  • Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice
  • Current certification by a national body such as ANCC, NBCHPN, or AANP
  • Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation
  • Current CPR certification required
Preferred Qualifications:
  • Palliative Care ARNP certification
