Patient Navigation Specialist

We are seeking a skilled and compassionate Oncology Nurse Navigator 2 to join our team at Rush. As a key member of the Cancer Center, this RN will play a vital role in promoting effective care coordination and patient navigation. The ideal candidate will have strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
In this role, you will be responsible for assessing patients' psychosocial, financial, and clinical needs, developing plans of care, and facilitating referrals and transitions of care. You will also serve as a liaison between patients, physician practices, the hospital, and the community, providing education and support to patients and families as needed. The salary for this role is approximately $80,000 - $100,000 per year, depending on experience.
Key Responsibilities
* Establishes caring therapeutic relationships with patients and families
* Assesses patients for social determinants of health and develops plans of care
* Serves as a liaison between patients, physician practices, the hospital, and the community
* Provides education and support to patients and families
* Facilitates referrals and transitions of care
