  • We are seeking a dedicated Licensed Practical Nurse to provide quality patient care and perform technical skilled care in the patient's home.

The LPN will be responsible for providing services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other members of the healthcare team.

Key Responsibilities:

  • Completes progress notes for each visit and submits progress notes to the agency daily.
  • Observes and reports patients' nursing needs, instructs patients' families in patient care, and maintains a healthy environment.
  • Observes, records, and reports patients' response to treatment and changes in patients' condition.

Requirements:

  • Current LPN licensure in state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
