Key Responsibilities:
- Collaboratively oversee the care progression of patients alongside the healthcare team, utilizing evidence-based practice guidelines.
- Engage patients and the care team in managing key patient outcomes, including disease management, length of stay, and transitions to appropriate post-acute settings.
- Advocate for patients by focusing on the overall patient experience beyond just the acute care episode.
- Ensure effective communication, optimal use of clinical resources, and management of patient flow throughout the care continuum.
- Serve as a knowledgeable resource for staff, providing ongoing education to colleagues, patients, and their families.
- Contribute to an environment where our mission is realized, patient-centered outcomes are achieved, and high-quality professional practice is maintained.

Qualifications:
Minimum Requirements:
- Current and valid New York State RN license
- Bachelor of Science in Nursing or a commitment to pursue a defined educational plan demonstrated by ongoing coursework
- A minimum of 2 years of experience in an acute care setting

Preferred Qualifications:
- At least 1 year of experience in case management or utilization management
- Certification in Case Management (CCM) or as a Certified Professional in Healthcare Management (CPHM)
- Knowledge of discharge planning and community resources
