Position Overview:


The Registered Nurse Case Manager (RN CM) is entrusted with the essential duty of overseeing the care coordination for residents in assisted living facilities.

The RN CM plays a pivotal role in guiding and supporting the care staff team, serving as a vital resource for staff while ensuring that residents, families, and staff are well-informed about ongoing care.

Central to this role is the management of Individualized Service Plans (ISP) for residents, ensuring that care staff work plans are in harmony with these plans.

The RN CM is responsible for liaising with resident physicians to confirm that physician orders align with the ISP and Medication Administration Records (MAR), as well as communicating with families regarding any changes in care.


Educational Qualifications:
Associates' Degree in Nursing (RN)

Current and unrestricted NY state RN license

Experience Requirements:
Minimum of 2 years of successful nursing experience

Experience working with the geriatric population is essential

Background in case management is preferred

Experience in assisted living settings is advantageous

The RN CM collaborates with the Terrace Director to oversee daily operations and assumes leadership responsibilities in the Director's absence.


Provides direction and support to the care staff team, ensuring that they have the necessary resources and information to deliver quality care.


In partnership with the Terrace Director, the RN CM develops care staff work plans that align with the ISP's.

The RN CM is also responsible for effective communication with resident physicians to ensure that orders are consistent with the ISP and MAR, and for keeping families informed of any significant changes.

Ensures compliance with regulatory standards.

Facilitates the admission process for new residents by conducting initial assessments and determining eligibility in accordance with established standards.

Reviews admission documentation.

Participates in management meetings to coordinate new resident transitions.

Organizes new resident records


Creates initial Individualized Service Plans and updates them at least every six months, or as residents' needs evolve.

Schedules medical appointments and transportation, or delegates these tasks to appropriate staff.

Ensures that case management documentation is current, comprehensive, and accurate.

Addresses concerns from families and residents promptly, escalating issues to the Director as necessary.

Monitors changes in resident needs and keeps all relevant parties informed.

Facilitates weekly interdisciplinary case management meetings.

Administers medications

Compensation:
$46.75 per hour

Compensation is determined based on experience, skills, and educational background.

Exempt positions under the Fair Labor Standards Act (FLSA) will receive compensation within the stated hourly rates.

The pay range may vary based on location.
