Position Overview:
The Registered Nurse Case Manager (RN CM) is responsible for overseeing the care coordination of residents in an assisted living environment.
The RN CM plays a crucial role in guiding the care staff team, acting as a resource for staff, and providing essential information to residents and their families.
They are tasked with developing and maintaining Individualized Service Plans (ISP) for residents, ensuring that care staff align their work plans with these ISPs.
Additionally, the RN CM communicates with resident physicians to ensure that all medical orders are consistent with the ISP and Medication Administration Records (MAR), and keeps families informed of any changes.
Educational Qualifications:
Associates' Degree in Nursing (RN)
Current and Unencumbered New York State RN License
Experience Requirements:
Minimum of 2 years of nursing experience
Experience working with the elderly population is essential
Background in case management is preferred
Experience in assisted living settings is advantageous
Key Responsibilities:

Acting as an advocate and liaison for residents
The RN CM supports the management of daily operations and assumes leadership responsibilities in the absence of the Director.
They provide direction and support to the care staff team, ensuring that they have the necessary resources and information.
In collaboration with the Director, the RN CM establishes care staff work plans that are in line with the ISPs.
The RN CM is responsible for liaising with resident physicians to ensure that medical orders are accurately reflected in the ISPs and MAR, and for updating families as necessary.
They assist in ensuring compliance with relevant regulations.Manage and coordinate the admission process for new residents by conducting initial assessments and verifying eligibility in accordance with established standards.
Review and process new admission documentation.
Participate in management meetings to facilitate the transition of new residents.
Prepare new resident files
Develop initial Individualized Service Plans and ensure they are reviewed and updated at least every six months or as resident needs change.
Ensure that case management documentation is thorough, accurate, and up-to-date.
Address any concerns raised by residents or families and escalate issues to the Director as necessary.
Stay informed about changes in resident needs and communicate these to families and physicians as required.
Organize and lead weekly interdisciplinary case management meetings.
Administer medications as required.
Compensation:
$46.75 per hour
Compensation is determined based on experience, skills, and educational background.
Exempt positions under the Fair Labor Standards Act (FLSA) will receive compensation within the stated hourly rates.
