  • Utilize evidence-based practice guidelines to inform care decisions and improve patient outcomes
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
  • Provide education and support to patients and families to promote health and wellness
  • Participate in quality improvement initiatives to enhance patient care and safety

Requirements:

  • Current and valid NYS RN license
  • Bachelor of Science in nursing, or willingness to pursue defined plan for matriculation in program demonstrated by ongoing coursework each semester
  • 2 years of experience in acute setting
  • Preferred requirements: 1+ year of experience in case management or utilization management, Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM)
